Safety Sitrep | Warranted Risk: Looking Beyond Probability and Severity in Public Safety Aviation

Posted by Admin

Government, military, and public safety aviation exist because society occasionally needs professionals to do what others are unable or unwilling to do, all while accepting elevated levels of known risk. [Read More...]

Safety Sitrep |Who’s the Best Pilot You Ever Saw?

Posted by Admin

I graduated from high school in 1983, inspired to become a pilot by the popular books and movies of the time. Like Craig Thomas’, Firefox Down and Tom Wolfe’s, The Right Stuff and their movie adaptations. In The Right Stuff, Gordon Cooper, played by a young Dennis Quaid, repeatedly asks, “Who’s the best pilot you ever saw?” His predictable reply: “You’re looking at him.” It’s a memorable line, filled with swagger, charisma, and a little cringe. Most of us have flown with someone like that.  [Read More...]

VAI Safety IAC to Focus on ADM at VERTICON 2026 | Start with Critical Preflight Conversations

Posted by Admin

At VERTICON 2026, the VAI Safety Industry Advisory Council (IAC)  leans hard into one of aviation’s most consequential skill sets: aeronautical decision-making (ADM). It starts where many safety outcomes are quietly won or lost during preflight conversations. [Read More...]

Safety Sitrep | The Art of Saying No

Posted by Admin

Accidents have occurred due to pilots’ failure to resist customer pressure to fly when operational, weather-related, or technical conditions were unsafe. Pilots may feel compelled to meet customer expectations, even when doing so compromises safety. Understanding how to handle such situations safely and effectively is crucial to reducing risk and preventing accidents. [Read More...]

Don’t be a Zombie

Posted by Admin

On a pleasant morning in March 1991, I was transiting back to my Intercoastal City, Louisiana base to pick up some newspapers for my offshore energy crew. I was alone in my mighty Bell 206B Jet Ranger and did not realize just how tired I was. You know that feeling you sometimes get in a long, not-so-interesting lecture, when you catch your head bobbing a bit. That’s what I had. It just took one head tilt, then I let go of the cyclic. Predictably, it fell forward as I entered a brief but alarmingly steep dive.  [Read More...]

Trust Is The Heart of Safety Decision Making

Posted by Admin

Imagine a twisted world where reporting a mistake doesn’t lead to a lesson learned, but to a lesson avoided—because no one felt safe enough to speak up. That's not just a missed opportunity, it’s a hidden risk waiting to become a headline. [Read More...]

The Importance of Safety Reporting

Posted by Admin

Imagine you're at the final question of the gameshow “Who Wants to be a Flight Safety Millionaire” (or whatever it might be called in your country). You've used all your lifelines. You ask the audience a question about flight preparation, and your CEO helped you at the $250,000 point by actually giving a right answer on Just Culture.  Now, you have a shot at the big money. You used your 50/50 and you're left with two options for this question: "Why do you report safety occurrences and hazards in your organization? [Read More...]

Securing Loose Equipment in the Cockpit is a Healthy Obsession

Posted by Admin

In the mid-1980s, during my first tour as a young and inexperienced U.S. Army AH-1F Cobra pilot in Germany, I went through a near-catastrophic event that forever changed my approach to cockpit safety. It happened during a routine but fun training mission where I was performing low-level flight maneuvers in a loose trail formation with several other Cobras. I was at the controls in the back seat, flying in the trail position, focused on terrain using the peaks and valleys to try to remain clear of a notional surface-to-air missile threat. It didn’t get much better than this for an easy day of flying.  [Read More...]

A Proposal for a Helicopter Safety Rating Scheme

Posted by Admin

Questions: How to incentivize installation of systems and equipment having safety benefits in the current fleet? How to create a positive dynamic to enhance safety? Are there successful initiatives in other industries that could inspire the rotorcraft community? [Read More...]
