On July 16, 2016, about 1315 eastern daylight time, a Schweizer 269C-1, N6148D, was substantially damaged after it experienced ground resonance while on a taxiway at the Tom B David Field Airport (CZL), Calhoun, Georgia. The commercial pilot and a commercial pilot rated passenger were not injured. The helicopter was being operated under the provisions of 14 Code of Federal Regulations Part 91 as a personal flight. Visual meteorological conditions prevailed and no flight plan was filed for the local flight, which originated about 15 minutes earlier from CZL.

ORLANDO, Fla., -- Sikorsky, a Lockheed Martin Company (NYSE: LMT), will build 12 additional HH-60W Combat Rescue Helicopters (CRH) following a second Low Rate Initial Production (LRIP) contract award by the U.S. Air Force, known as Lot 2, and valued at over $500 million. The award follows a string of significant program milestones in 2019, including first flight, a Milestone C decision by the Air Force, and award of the first Low Rate Initial Production (LRIP) contract for 10 aircraft. The commercial pilot reported that during an aerial application flight, he sprayed a row on the field and climbed to avoid wires on the south side of the field. He then made a right turn and began his descent back to the field. After he cleared the wires, he entered the field on the spray line and began raising the collective. He noticed that the torque gauge was responding, however, the rate of descent was not being halted and the helicopter subsequently impacted the ground and came to rest on its right side.

The student pilot reported that he hovered the helicopter to the parking pad and landed with the helicopter's nose facing north. The skid placement on the parking pad was incorrect, and the flight instructor directed the student to reposition the helicopter about 3 ft to the left. The student brought the helicopter to a hover and repositioned it 3 ft to the left.

The pilot reported that, after the fourth aerial application flight of the day, he landed and positioned the helicopter near the autoloader boom truck. He reduced engine power to flight idle, and the ground crew began the loading process. He verified that the collective position was full down and the cyclic friction was on, and he then exited the helicopter to stretch while maintaining one hand on the cyclic. The pilot reported that, immediately after, while standing next to the helicopter, he heard two "loud bangs." He immediately boarded the helicopter, the ground crew secured the truck's boom, and the pilot shut down the engine.

The pilot reported that, while making a video of a boat, he flew the helicopter with the doors removed about 20 ft above the water and 3/4 mile offshore and saw seabirds ahead crossing right to left but he thought they would be not be a problem. Shortly after seeing the birds, he initiated a right turn to follow the boat, and while he was looking over his right shoulder, he felt an object, which he believed was a seabird, strike him in the back of the head. While dazed from the head strike, he flared to reduce speed, and the helicopter settled into the water. He then applied left lateral cyclic to stop the main rotor blade's rotation, and the helicopter rolled left and inverted in the water.

On August 10, 2018, at 0640 mountain daylight time, an unregistered Dà-Jiang Innovations (DJI) Mavic Pro small unmanned aircraft system (sUAS, commonly known as a drone), operated by an individual, collided with a Lindstrand 105A hot air balloon, N1410C, operated by Elevated Ballooning as a revenue passenger sightseeing flight under the provisions of 14 Code of Federal Regulations (CFR) Part 91. The collision occurred in the vicinity of the Driggs-Reed Memorial Airport (KDIJ), Driggs, Idaho. The balloon was not damaged, the sUAS was substantially damaged, and there were no injuries or ground damage. Visual flight rules (VFR) conditions prevailed at the time of the incident.
