NTSB Final Report: Moab, UT

The pilot reported that, he was initiating a landing to a ledge. As the landing skids contacted the ground, the pilot felt the helicopter settle tail first. Startled by this, he abruptly raised the collective, unaware that he was holding forward cyclic input. As power was applied, the rear skids lifted off the ground first, creating a nose low attitude. The helicopter subsequently rolled over onto its right side substantially damaging the fuselage. The pilot reported that there were no preaccident mechanical failures or malfunctions with the helicopter that would have precluded normal operation.

Enstrom Helicopter First Flight

Enstrom is excited to announce the flight of the first new 480B Helicopter after reopening just eight months ago. The turbine-powered 480B is the 1317th Enstrom helicopter built, and the 255th 480 series helicopter. This aircraft wasn’t an unfinished left-over airframe, but was built using components procured, machined, fabricated, and assembled onsite.

NTSB Final Report: Camp Dwyer, Afghanistan

During the approach to the airfield at Camp Dwyer, the helicopter entered an uncommanded left yaw while approaching its destination. During the subsequent emergency landing, the helicopter impacted the ground and rolled on its right side, resulting in serious injuries to the three occupants and substantial damage to the helicopter. Investigation found there was no evidence of a preimpact structural failure, nor a main or tail rotor system failure, nor a malfunction of either engine.
