The flight instructor stated that the accident flight was an introductory flight for a new student pilot. He stated that during an introductory flight, it was normal for a student pilot to take the cyclic control and get familiar with the “feel” of the helicopter. During the accident pilot, the student pilot hovered the helicopter at 10 ft above ground level for about 10 minutes with no anomalies, then suddenly, the student pilot pushed the cyclic forward and to the right “very fast.” The flight instructor could not regain control before the main rotor contacted the ground. The helicopter rolled to the right and came to rest on tits right side, sustaining substantial damage to the main rotor mast, tail rotor, and fuselage. The flight instructor stated there were no preimpact mechanical malfunctions or failures of the helicopter that would have precluded normal operation.

The Zambian military had previously purchased Enstrom Turbine 480Bs with delivery expected in the near future pending aircraft completion. The helicopter was operating at a remote, unimproved landing zone (LZ) with a ground crew consisting of firefighters. They were conducting aerial firefighting training with an external load consisting of a 50 ft longline and a water bucket. The LZ consisted of about 2.5 ft tall prairie grass, with the tail of the helicopter going up slope. During the landing to the LZ, the tail rotor impacted a backpack on the ground. Marignane, 31 October 2023 – Airbus Helicopters and Naval Group, in collaboration with the French Armament General Directorate, DGA (Direction génerale de l’armement), and the French Navy, have tested the SDAM demonstrator (Système de Drone Aérien Marine/ Naval Aerial Drone System) from a multi-mission frigate (FREMM). The trials took place on board the French Navy frigate, Provence in the Mediterranean Sea between the 2nd and the 9th of October. The vessel had previously been adapted by Naval Group to operate the SDAM. These sea trials were arranged to demonstrate the system’s high performance from an operational warship and the SDAM’s capabilities for surveillance and intelligence missions.

Leonardo announced an order today for three AW139 medium twin-engine helicopters by Monroe Country in Florida. These aircraft will be operated by Trauma Star, a joint effort of Monroe County Fire Rescue and the Monroe County Sheriff’s Office and will provide air ambulance support across the Florida Keys from Lower Keys Medical Center and the Florida Keys Marathon International Airport. These AW139s will be delivered from Leonardo’s FAA-Certified Part 21 Production Facility in Philadelphia, Pennsylvania, between the end of 2024 and early 2025, after which it will undergo dedicated mission customization before replacing the County’s existing fleet.
