NTSB Final Report: Waukon, Iowa

The pilot was conducting an agricultural application pass when the helicopter struck a powerline crossing the cornfield being sprayed. He did not see the line until exiting the helicopter after the accident. Earlier in the morning he had surveyed the area for obstacles but did not observe any lines crossing the field. The pilot commented that his initial survey pass was completed toward the east with the sun “low in the sky and in [his] eyes.” He noted that there were no issues with the helicopter before contacting with the power line. The helicopter sustained substantial damage to the main rotor system, the fuselage, and the empennage.

FAA Issues Certificate Of Airworthiness For The First Type Certified S-70M™ Black Hawk® Helicopter

STRATFORD, Connecticut, Dec. 7, 2021 – The Federal Aviation Administration (FAA) has completed its certification of the S-70M Black Hawk helicopter — a new type designation — by issuing Sikorsky, a Lockheed Martin company (NYSE: LMT) a Restricted Category Special Airworthiness Certificate. FAA certification, and the establishment of a pilot type rating, are expected to broaden the market for the military-designed helicopter by allowing civil and commercial operators in the United States to purchase new Black Hawk aircraft direct from the factory.

NTSB Preliminary Report: Perry, OK

On November 28, 2021, about 1658 central standard time, a Bell 206B helicopter, N59600, was destroyed when it was involved in an accident near Perry, Oklahoma. The commercial pilot sustained fatal injuries and the student pilot sustained serious injuries. The helicopter was operated as a Title 14 Code of Federal Regulations (CFR) Part 91 personal flight.
