On August 3, 2020, at 1941 eastern daylight time, an Airbus EC130-B4 helicopter, N55GJ, was substantially damaged when it was involved in an accident near Knoxville, Tennessee. One passenger was fatally injured, and the pilot and two additional passengers were not injured.

On June 8, 2022, about 1726 Hawaii-Aleutian standard time, a Bell 407 helicopter, N402SH, sustained substantial damage when it was involved in an accident near Kalea, Hawaii, on the island of Hawaii. The pilot and two passengers sustained serious injuries, and three passengers sustained minor injuries. The helicopter was operated as a Title 14 Code of Federal Regulations Part 135 on-demand air tour flight.

The solo student pilot reported that, just after liftoff and in a low-altitude hover, the helicopter “swung” to the left. The student pilot reported that he “overcorrected” with right pedal and aft cyclic inputs. The helicopter began to spin, and the student pilot pulled up to keep from hitting the ground. The student pilot reported remembering hitting the ground and rolling the throttle off.
