NTSB Final Report: Sanford, FL

Posted by Admin

As part of a law enforcement simulation exercise, the helicopter pilot was orbiting near the departure airport between 200 to 300 ft above ground level, at 20 to 40 knots indicated airspeed. The pilot described that while the helicopter was in a left orbit the airspeed decreased to near 0. At that time, the helicopter began to descend in a vortex ring state (VRS). Due to the low altitude, the pilot was not able to recover from VRS and entered an autorotation to a grass area on the airport. The helicopter subsequently landed hard and came to rest upright in the grass area resulting in substantial damage to the helicopter’s tail boom. The pilot reported that there were no preimpact mechanical malfunctions or failures of the helicopter.

NTSB Final Report: Miami, FL

During an air tour flight at night, the pilot of the helicopter noted that the oil pressure and temperature gauges were displaying indications that were in the “red area,” so he decided to perform a precautionary landing to a closed helipad. During the precautionary landing, while maneuvering the helicopter just prior to touchdown, the tail rotor contacted a plastic pipe that protruded up about 2 ft from the ground. The pilot further described that it was very dark and that he was unable to see the pipe. Following the tail rotor strike, the helicopter landed hard, deforming the landing skids. The helicopter sustained substantial damage to the vertical stabilizer during the accident.

The pilot of the helicopter reported that while flying at an indicated altitude around 200 to 300 ft, while on approach to land at an off-airport landing zone, he saw a black “drone” [unmanned aerial system (UAS)] in front of his windscreen. He attempted to avoid the UAS but was unable and the UAS impacted the main rotor resulting in substantial damage to a main rotor blade.

NTSB Final Report: Immokalee, FL

Posted by Admin

The flight instructor stated that the accident flight was an introductory flight for a new student pilot. He stated that during an introductory flight, it was normal for a student pilot to take the cyclic control and get familiar with the “feel” of the helicopter. During the accident pilot, the student pilot hovered the helicopter at 10 ft above ground level for about 10 minutes with no anomalies, then suddenly, the student pilot pushed the cyclic forward and to the right “very fast.” The flight instructor could not regain control before the main rotor contacted the ground. The helicopter rolled to the right and came to rest on tits right side, sustaining substantial damage to the main rotor mast, tail rotor, and fuselage. The flight instructor stated there were no preimpact mechanical malfunctions or failures of the helicopter that would have precluded normal operation.
