NTSB Final Report: DeLand, FL

Two pilots were attempting to reposition the vintage, experimental-exhibition category helicopter into a hangar to protect it from hail damage before a severe cold front arrived. The pilots initially attempted to land the helicopter on a movable skid, but because the skid was too close to a building (given the wind conditions), the pilots landed the helicopter before one of the pilots disembarked and moved the skid. The accident pilot then took off in the helicopter and flew in a roughly circular pattern to approach the skid for landing. During the flight the helicopter “developed a severe shake,” so the pilot decided to immediately land the helicopter on the grass below. During the landing, the helicopter’s tail rotor struck the ground, substantially damaging it and its mount/drive system. The pilot reported that there were no preimpact mechanical malfunctions or failures of the helicopter that would have precluded normal operation. He further described that at the time of the accident the wind velocity was about 30 knots, and “I wish it had not been so windy, with the wind whipping around the building. If the skid had been in an area of clear air, it would have been fine.”

NTSB Final Report: Caseville, MI

The pilot reported that while on a local flight he noticed a squall line approaching and elected to return to the private strip from which he had departed. During the approach, rain intensity increased, and the wind began gusting to about 15 knots. The pilot landed the helicopter, lowered the collective, and reduced power. While the main rotor was turning about 420 rpm, the helicopter suddenly rolled over onto its right side, resulting in substantial damage to the main rotor system and fuselage.

NTSB Final Report: Holley, NY

The pilot of the helicopter was practicing hovering and maneuvering just above the surface of the ground, around a cone. The pilot stated that because he was focused on the cone and not the helicopter’s altitude, the rear portion of the right skid contacted the ground resulting in dynamic rollover. The helicopter’s tailboom was substantially damaged during the accident. The pilot reported there were no preimpact mechanical malfunctions or failures of the helicopter that would have precluded normal operation.

NTSB Final Report: Lakeland, FL

The pilot reported that during the approach to land, he initiated a left turn to align with the wind and landing pad when the helicopter entered an uncommanded right spin. He attributed the loss of control to an 18-knot tailwind and described the event as consistent with a loss of tail rotor effectiveness. He was unable to regain control, and the helicopter descended in a spiraling right turn before impacting the ground. A post-accident review of photographs by the NTSB investigator confirmed that the helicopter sustained substantial damage to the fuselage.

NTSB Final Report: Hartford, IL

The purpose of the public relations flight with the air medical helicopter was to perform landing zone training with a fire department. Prior to the flight, the pilot discussed the confined area landing zone characteristics with another company pilot, and he studied satellite imagery of the confined area landing zone. The flight manifest and the flight risk assessment tool were completed and submitted to the operational control center, which then issued a flight release after approval. Prior to initiating the approach, the pilot performed a high reconnaissance to identify the confined area landing zone and the obstacles which consisted of a tree and power lines. The pilot then performed a low reconnaissance while simultaneously performing a steep approach. The helicopter landed in the vacant lot, consisting of a flat grass field, across the street from the fire department.
