The two pilots and four passengers were fatally injured. The helicopter was operated by Orbic Air, LLC, as a Title 14 Code of Federal Regulations Part 135 on-demand flight.

On January 20, 2024, about 2323 central standard time, a Bell 206L-3 helicopter, N295AE, was substantially damaged when it was involved in an accident near Hydro, Oklahoma. The pilot, flight nurse, and flight paramedic were fatally injured. The helicopter was operated as a Title 14 Code of Federal Regulations Part 135 positioning flight.

The pilot of the helicopter reported that while flying at an indicated altitude around 200 to 300 ft, while on approach to land at an off-airport landing zone, he saw a black “drone” [unmanned aerial system (UAS)] in front of his windscreen. He attempted to avoid the UAS but was unable and the UAS impacted the main rotor resulting in substantial damage to a main rotor blade.

On January 11, 2022, about 1255 eastern standard time, a Eurocopter EC135 P2+, N531LN, was substantially damaged when it was involved in an accident in Drexel Hill, Pennsylvania. The airline transport pilot was seriously injured. The two medical crewmembers and the patient were not injured. According to two witnesses, the pilot of the gyroplane started his takeoff roll with the rotor in a horizontal position. Both witnesses stated that the gyroplane’s take off roll was longer than usual. When the pilot pitched the rotor up, the gyroplane pitched up, climbed about 30 to 40 ft above the ground, rolled to the left, and subsequently impacted the runway. The left main landing gear separated from the gyroplane after impacting the runway, and the gyroplane departed the runway to the left. The gyroplane came to rest about 1,400 ft from its departure point. The pilot was seriously injured. During the runway excursion, the gyroplane’s rotor impacted the ground resulting in substantial damage to the rotor, horizontal stabilizer, vertical stabilizer, and rudder.
