NTSB Final Report: Gila Bend, AZ

On April 15, 2022, about 0837 mountain standard time, a Robinson R22 helicopter, N7516G, sustained substantial damage when it was involved in an accident near Gila Bend, Arizona. The student pilot was fatally injured. The helicopter was operated as a Title 14 Code of Federal Regulations Part 91 instructional flight.

NTSB Final Report: Chandler, AZ

On October 1, 2021, about 0740 mountain standard time, a Piper PA-28-181 airplane, N2868H, and a Robinson Helicopter Company R22 helicopter, N412TL, were involved in a midair collision near Chandler, Arizona. The airplane sustained minor damage and the flight instructor and student pilot onboard were not injured. The flight instructor and student pilot onboard the helicopter were fatally injured, and the helicopter was destroyed. Both aircraft were operated as Title 14 Code of Federal Regulations Part 91 instructional flights.

NTSB Final Report: Benson, AZ

The pilot of the gyroplane reported that, typically after reaching a speed of about 45-50 mph during the takeoff roll and just before takeoff, the nosewheel becomes light. He stated that for the accident flight the nose became light as expected, but then suddenly dropped, and the nose gear contacted the runway hard. The gyroplane veered left, and he depressed full right rudder. The main rotor impacted the runway, and the gyroplane departed the runway and came to rest on its side, sustaining substantial damage to the fuselage, tail boom, and rotor system.

NTSB Final Report: Mesa, AZ

While conducting an instructional flight, the flight instructor reported that, he assumed control of the helicopter to demonstrate a left 180° autorotation, and the maneuver began normally. Part way through the maneuver, the pilot noticed that the helicopter was lower in altitude than he expected. He reported that he applied right cyclic and aft cyclic, attempting to slow the rate of descent. The helicopter landed hard and skidded to a stop. When stopped, the flight instructor and the student pilot exited the helicopter, and a postimpact fire ensued. The tail boom and fuselage sustained substantial damage. The pilot reported that there were no preaccident mechanical malfunctions or failures with the helicopter that would have precluded normal operation.
