ACSF's mid-year ASAP data puts operational risk over mechanical

It's largely a given that the largest safety risks in aviation today are typically found in the cockpit and the airspace around the aircraft, not the airframe or engine. The current data, along with lessons learned, is broken down in the recently released 2026 ASAP (Aviation Safety Action Program) Mid-Year Review by the Air Charter Safety Foundation (ACSF). The report uses a pool of 432 aviation safety events that took place in the first six months of 2026 as its foundation. Altitude and traffic events lead the 2026 data The report states that altitude deviations, traffic proximity events and crew coordination issues made up 41% of reported safety events. Altitude deviations led all categories at 16%, followed by traffic proximity events at 14% and coordination/communication issues at 11%. Aircraft mechanical issues accounted for only about 10% of reported events across airframe, powerplant, and structure categories combined. Why operational risk outweighs mechanical risk While many aviation discussions focus on equipment reliability, the data suggests that an operator's attention may be better directed toward other categories. Operational risks like altitude management and traffic awareness are often mitigated differently than maintenance risks. Instead of changing a part or performing maintenance, fixing an operational issue requires training, data analysis, and procedures that can take more time and effort. What operators need to know This data has direct implications for charter operators, fractional programs, and management companies as they evaluate these areas when assessing their own safety culture and operational discipline. The findings show how investments in training, communication, and workload management can influence both safety performance and operational consistency. The larger takeaway is that safety investments may generate stronger returns when focused on human-performance and operational-execution risks rather than exclusively on mechanical exposure. The year-end ASAP numbers should provide a better snapshot of which direction the industry is headed. Time will tell whether operators heed these warnings or put the report on the shelf until next year.
