Bering Air Caravan crash: NTSB cites excess weight and cockpit workload

The NTSB released its final report on Bering Air Flight 445, a Cessna 208B Caravan which went down on approach to Nome, Alaska (PAOM) on February 6, 2025 with a pilot and nine passengers on board. None survived. The weather was worse than forecast and the airplane was exposed to large, supercooled droplets, the kind that typically build up ice quickly.Just prior to the accident, PAOM was temporarily closed while the airport deiced its runways. The single pilot of the Caravan was put in a holding pattern and tasked with rebuilding his approach. The airspeed dropped to 89 knots and the pilot made a nose-up input as the aircraft entered a stall. The result was a deepening stall from which the pilot could not recover.What the final report foundThe final report's probable cause was listed as inadequate airspeed management, task saturation and eroded situational awareness. Investigators recovered the avionics data which showed that the TKS system did its job as intended. It kept significant ice off protected surfaces and showed that the airframe held up its end of the bargain.That sets the table for two additional factors that operators need to be aware of — the Caravan was equipped with STC SA01213SE, the Aircraft Payload Extender III (APE III) system held by AeroAcoustics Aircraft Systems. That STC raises the maximum takeoff weight to 9,062 pounds from the Caravan's original 8,807 pounds. That payload increase does not apply to flight into known icing conditions. The POH supplement resets the limit to the aircraft's original capacity, which put this flight approximately 1,000 pounds over the limit for known ice. The board found that overweight condition causal: it raised the stall speed and shrank the margins he was flying within.The board's findings also pointed to a normalization of deviance in Bering Air's procedures, where departing overweight flights became routine and aircraft weights were underreported on load manifests. The report also faulted the FAA for not tightening oversight on a fast-growing operator that had expanded after a major competitor's bankruptcy. NTSB Chair Jennifer Homendy, who traveled to Nome, called the accident the result of "a series of preventable breakdowns that eroded critical safety margins."The lessons for operatorsThe 208's known-ice limit is real and the STC's payload caveat is something operators need to put front and center, along with adherence by pilots to the handbook's 95-knot minimum airspeed. Single-pilot workloads are always challenging when configuring for approach in icing conditions and the NTSB's recommendations are squarely aimed at mitigating the risks of these types of situations. The board specifically has called for most charter, air taxi, and fractional operators to train pilots specifically to prevent and recover from a loss of control and re-stressed the need for certificated dispatchers and wider flight-data monitoring across Part 135 flying.While the ruling doesn't technically change an operating rule, operators of the 208 or other single-pilot turboprops should use this occasion to check their dispatch math (especially with aircraft equipped with the APE III system) and review pilot procedures and training. The heart of the report is prevention, with the hope that the lessons learned from Flight 445 are not soon forgotten by pilots and operators.
