NTSB concludes pilot failed to maintain airspeed in 2023 crash that killed 2 in Oshkosh

The pilot's failure to maintain airspeed caused the crash of a T-6 Texan near Oshkosh, Wisconsin, during the week of AirVenture two years ago, killing two people. That's the probable cause listed in a final report released by the NTSB on Wednesday. It states that on July 29, 2023, the pilot, identified as 30-year-old Devyn Reilly of Guadalupe, Texas, took off from Runway 27 at Wittman Regional Airport (OSH) just before 9 a.m. 20-year-old Zach Colliemoreno was also on board. The aircraft, a North American AT-6D, made a right turn toward the north and continued for 3 miles before turning toward the east-northeast over Lake Winnebago, outside of the OSH Class D airspace. It climbed to an altitude of about 3,900 feet above mean sea level while over the lake. ADS-B data and winds aloft data showed that during the turns, the aircraft decelerated to 87 mph calibrated airspeed, and the calculated load factor increased. The aircraft's speed was consistent with its accelerated stall speed before it rapidly descended. A witness saw the aircraft in a flat spin to the right before it hit the water. It broke into pieces and came to rest at the bottom of the lake. The T-6 Texan was later recovered and examined. After examining the aircraft, the engine and related systems did not show any pre-impact anomalies. The NTSB determined that Reilly's failure to maintain airspeed while maneuvering resulted in the exceedance of the aircraft's critical angle of attack and an aerodynamic stall/spin. Reilly and Colliemoreno were both killed in the crash. The report states Reilly had more than 687 hours of flight experience, including 33 hours in the T-6 Texan. Reilly was the daughter of two-time Super Bowl champion Bruce Collie and co-founder of the Texas Warbird Museum.
