NTSB Final Report: Covelo, California

The pilot of the helicopter reported that, he was conducting a local area photography/video flight with passengers. Before departure he had removed the doors and provided a preflight safety briefing, which included briefing his passengers on securing their belongings. While in cruise flight, a passenger’s jacked departed the helicopter and became lodged on the synchronized elevator. The pilot simultaneously felt an impact and noted resistance against the cyclic control. A precautionary landing was made without further incident. The tail rotor drive shaft was substantially damaged.

NTSB Final Report: Ashland, Oregon

On January 23, 2019, at 1143 Pacific standard time, a Bell 206L-3, N8227J, was substantially damaged when it was involved in an accident near Ashland, Oregon. The pilot was fatally injured. The helicopter was operated as a Title 14 Code of Federal Regulations (CFR) Part 91 flight.

NTSB Final Report: Lancaster, Pennsylvania

According to the flight instructor, the purpose of the flight was to conduct hovering and pattern work maneuvers. After the completion of the flight, the student pilot was hovering and awaiting clearance to return to the ramp. The flight instructor said she was looking around to ensure the runway was clear and making a radio call, when suddenly, the helicopter pitched up. She attempted to gain control of the helicopter but was unsuccessful. The helicopter collided with the ground and rolled over. The flight instructor said she was not on the flight controls prior to the accident event.

NTSB Final Report: Fort Davis, Texas

The pilot reported that he had descended the helicopter to about 50ft above ground level along a ridgeline when he encountered a downdraft from an opposing ridgeline. He attempted to fly out of the downdraft by flying towards lower ground but did not have the power or airspeed to arrest the descent. The helicopter’s low rotor RPM came on as the helicopter impacted terrain and rolled onto its left side. The helicopter sustained substantial damage to the main rotor system, fuselage, and tail boom. The pilot reported that there were no preaccident mechanical malfunctions or failures with the helicopter that would have precluded normal operation.
