NTSB Final Report: McKinney, Texas

Posted by Admin

The pilot reported that he drove from his home to the airport not expecting to fly the helicopter due to high winds, however, after arrival at the hangar the winds were lighter than the forecast winds, so he conducted a preflight for a local flight. As the pilot raised the collective for takeoff and the helicopter climbed into an initial hover, the helicopter rolled to the left. The pilot tried to counter the roll but was unable because the cyclic control lock was still in place. The helicopter continued to roll left and impacted the ramp and a post impact fire ensued. The pilot successfully egressed the helicopter but suffered serious burn injuries. The helicopter sustained substantial damage to the main rotor system and the cabin. The pilot reported that there were no preaccident mechanical malfunctions or failures that would have precluded normal operation.

NTSB Final Report: Fort Davis, Texas

Posted by Admin

The pilot reported that he had descended the helicopter to about 50ft above ground level along a ridgeline when he encountered a downdraft from an opposing ridgeline. He attempted to fly out of the downdraft by flying towards lower ground but did not have the power or airspeed to arrest the descent. The helicopter’s low rotor RPM came on as the helicopter impacted terrain and rolled onto its left side. The helicopter sustained substantial damage to the main rotor system, fuselage, and tail boom. The pilot reported that there were no preaccident mechanical malfunctions or failures with the helicopter that would have precluded normal operation.

NTSB Final Report: Wichita Falls, Texas

Posted by Admin

The pilot reported that the purpose of the flight was to practice emergency procedures with a certified flight instructor (CFI). After completing four uneventful autorotations, they climbed the helicopter to an altitude of about 1,500 mean sea level and turned onto the final approach leg of the traffic pattern to conduct a ‘hydraulics off’ approach. Once established on final approach, the CFI reached down to turn off the hydraulic switch, and the engine lost complete power. The pilot immediately gave the flight controls to the CFI, and he prepared for an autorotation in a nearby field. During the descent, the helicopter impacted powerlines before it landed hard. The main rotor blade severed the tailboom, and the helicopter spun before it came to rest on its side.
