NTSB Report: Unsecured Fire Extinguisher Cover Damages EC135 Fenestron During Landing
How can one small piece of unsecured equipment cause substantial helicopter damage? In this NTSB accident review, we examine an EC135 air ambulance accident where rotor wash lifted a cloth fire extinguisher cover into the helicopter's fenestron during landing. Although the pilot landed safely and no one was injured, the aircraft sustained significant damage. The investigation determined the probable cause was the inadequate securement of the fire extinguisher cover, highlighting the critical importance of foreign object debris (FOD) prevention around helicopter landing areas.
